Pharmacy Benefit Mangers (PBMs), insurers, and other supply chain entities accounted for one quarter of every prescription drug dollar spent in the U.S. in 2023, compared to around 50 percent for manufacturers. This statistic shows the share of drug dollar retained by drug supply chain participants, as estimated in January 2025 by using 2023 data. The debate about drug pricing in the U.S. is far more pronounced than in other developed countries. Especially, manufacturers on one side and PBMs and other supply chain entities on the other are blaming each other for too high drug prices. Â
