*Maternal mortality refers to the death of a pregnant person during pregnancy or within 42 days after the pregnancy ends. Pregnancy-related mortality includes deaths during pregnancy or up to one year after the pregnancy ends.
Mortality rates were adjusted for random effects of states and counties, proportion of maternal age 35 years or older, obesity, chronic hypertension, pregestational diabetes, Social Vulnerability Index, and year of birth.Â

U.S. maternal mortality rates 2018-2021, by maternity care access
A U.S. report from 2025 found that there is an association between maternity care access and maternal and pregnancy-related mortality*. Maternal mortality (and pregnancy-related mortality) rates were both higher in counties that are maternity care deserts, compared to counties with full maternity care access. Roughly ** percent of all U.S. counties were designated as maternity care deserts as of 2024.
