The source adds the following information: "Pathology definition algorithm: Individuals with a Long-Term Condition during year n with ICD-10 codes for heart failure (HF), hypertensive heart disease, or hypertensive cardiorenal disease, and/or individuals hospitalized in MCO (medicine, surgery, obstetrics) during years n to n-4 for heart failure (primary diagnosis of one of the Medical Unit Summaries or related diagnosis), and/or individuals hospitalized during years n to n-4 for hypertensive heart disease with HF, hypertensive cardiorenal disease with or without specification, cardiac liver, or acute pulmonary edema (primary or related diagnosis of one of the Medical Unit Summaries) with an associated or related diagnosis of HF, and/or individuals hospitalized in MCO during year n for any other reason with HF as a complication or associated morbidity (associated diagnosis, or primary or related diagnosis of one of the Medical Unit Summaries), and/or individuals hospitalized in Follow-up and Rehabilitation Care during year n with HF as the main morbid manifestation, etiological condition, or associated diagnosis, (and without hospitalization for acute HF during year n)."
