* The underlying cause of death was coded according to the 6th Revision of the International Classification of Diseases (ICD) in 1950, the 7th Revision in 1960, the 8th Revision in 1970, and the 9th Revision from 1980 to 1998. Starting with data from 1999, the cause of death is coded according to the ICD-10 coding system.
All rates are age-adjusted. Age-adjusted rates are calculated based on the standard population of the year 2000. Before 2003, age-adjusted rates were calculated from standard proportions per million using rounded population figures. From 2003 data onward, unrounded population figures are used to calculate age-adjusted rates.
