Supplementary notes
Low-risk cesarean is defined as singleton, head first, term (37 or more completed weeks) first births that were cesarean deliveries.
Preterm births: before 37 completed weeks of gestation based on the obstetric estimate.
Late preterm births: at 34–36 completed weeks of gestation based on the obstetric estimate.
