Health care systems remain a central pillar of health governance. The management of medical resources strongly shapes accessibility and response capacity in a sector that greatly impacts societal wellbeing. As a result, governments worldwide are under pressure to effectively allocate, distribute, and oversee financial and material health care resources. Yet, high levels of spending and infrastructure capacity do not necessarily translate into better health outcomes. This leads to considerable challenges when comparing systems within a sector that is characterized by high complexity and marked regional inequalities.
Global performance and rankings
Global rankings are contingent on what is measured. According to an international ranking, Japan, Italy, and Iceland are the countries with the strongest health care systems in the world when considering medical resources, medicine availability, and government readiness, with index scores ranging between 74 and 79, out of a total of 100 possible points. In contrast, Oman, South Africa, and Hong Kong rank among the countries with the lowest scores, lagging by around 15 points.Â
However, when focusing on the capacity to treat and cure diseases, Finland, Belgium, and Japan top the list, while South Sudan, Madagascar, and Afghanistan place last. Once health outcomes are factored in, Asian countries lead the ranking, with Singapore, Japan, and South Korea achieving the highest scores. By comparison, countries such as the Central African Republic, South Sudan, and Chad rank lowest. This last dimension is the most complex. Health outcomes are strongly influenced by health care systems. However, they also depend on an array of socio-economic and environmental constraints all relevant when dealing with comparisons.
Public perception of health care systemsÂ
These marked contrasts between countries and the observed gaps between spending, infrastructure, and outcomes highlight a dual reality. While considerable improvements in health care have been achieved in recent decades, challenges remain regarding access and affordability on a global scale. In fact, according to a 2025 survey conducted across 30 countries around the world, nearly half of respondents stated that access to treatment and long waiting times are among the most pressing issues facing health care systems. A similar share was observed for insufficient staffing, while around a third of respondents agreed that cost of accessing medical care remains a major concern.Â
Overall, strengthening health care systems requires coordinated and multilayered efforts that consider not only resource allocation and management, but also the particular needs of each population. This includes differences between countries, along with those present among demographic segments.Â
