The Universal Health Care (UHC) Act of 2019 marked a major turning point for Philippine healthcare. Today, nearly all Filipinos are automatically enrolled in the National Health Insurance Program (NHIP), administered by PhilHealth—a system now covering about 90 percent of the population. Current health spending has grown rapidly, reaching 1.44 trillion in 2024, a 17% increase from the previous year and nearly six percent of the nation's GDP.
UHC drives progress but gaps persist—too many communities remain underserved, and costs keep rising. Continued investment in more clinics, staff, equipment, and expanded insurance benefits is key to delivering accessible, quality healthcare for all Filipinos.
State of health
At 70 years, the Philippines has one of the lowest life expectancy at birth among ASEAN countries in 2023. Heart disease and cancer remain the primary contributors to mortality, which can be attributed to the high fat and salt content of the Filipino diet. This also contributes to the rising levels of obesity, especially between the ages of 20 and 59. UHC aims to reduce out-of-pocket healthcare costs, expand facilities, and increase medical staff, especially in remote areas. Yet, many patients still face financial strain as soaring healthcare costs, with some choosing alternative cures instead of professional help. The current healthcare approach leans more toward curative rather than preventive care. In fact, spending on preventive care accounted for just about 10 percent of total current health spending in 2024. While PhilHealth provides broad access to inpatient and select outpatient services, specialized treatments and many private services remain expensive or uncovered.Rural vs. urban divide: facilities and professionals
Although the country now hosts almost 42,000 health facilities, most are barangay health stations with limited equipment, especially outside cities. Urban areas have better-resourced hospitals and a far higher concentration of health workers; Metro Manila boasts about eight times more barangay health workers per person than the national average.
